Nitrous Oxide

To be eligible for enrollment individuals must:

have a current Oklahoma Dental Assisting Permit, Oral Surgery Assistant Permit -or- Dental Hygiene License,
have a CURRENT* Healthcare Provider or Professional Rescuer Level CPR card or eCard from an IN-PERSON skills testing provider (Online skills testing is not allowed)

The enrollment fee must be paid in full before you will be given a spot in the course.

You will not be considered for enrollment if you do not have the above-mentioned form of CPR*

*If your CPR card EXPIRES BEFORE YOUR COURSE DATE, your application will NOT be accepted. Please obtain a CPR that will be valid at the time of your course before enrolling (NO EXCEPTIONS). LETTERS FROM THE INSTRUCTOR WILL NOT WORK IN PLACE OF THE CARD.

Applications will be processed in the order they are received. Spots in the course will not be secured until applications are approved and payment is received. Once your application has been approved and payment is received, you will receive a final confirmation email. Applications are only processed during business hours.

Is your CPR a BLS for the Healthcare Provider or BLS for the Professional Rescuer?*

Did your CPR course have a hands-on, IN-PERSON, skills test?*

What is the expiration date on your CPR? (Choose last day of expiration month i.e. 1/31)*

You will only be allowed to enroll in course dates that take place BEFORE your CPR expires.
